I give permission for first aid to be carried out on my behalf by a trained first-aider.
 
I consent to any emergency medical treatment necessary during the running of the club
I will ensure my child has the correct football kit, will arrive and be collected on time, behave and be respectful to others and follow the instructions of the coaches.

I understand and accept that whilst all reasonable and responsible care will be taken any personnel authorised by them, or myself, nor taken facility at which the course is being held, will be held responsible for any loss or injury by, or too, the applicant, however so caused.
Photographic release. I give Cre8 Football and its representative’s permission to use photographs or video that includes my child in any and all media products for promotion, art, advertising, editorial or other purpose. This may include but is not limited to social media, newsletters both print and email, posters, and web pages.
